Site Risk & Action Tracker

A site management project by Thodah Hovor, MLS(ASCP)

How I would manage a portfolio of sites as a CRA or site manager: score each one on the risks that matter most to patients and data, explain why it's flagged, act on the root cause, and verify the fix. Lab-side red flags, the ones most monitors miss, are built in.

My approach

1. SpotTrack key risk indicators for every site and focus effort where safety and endpoint data are most at risk.
2. DiagnoseAsk why, not just what. A late sample is a symptom; the Friday draw with no courier is the cause.
3. ActAgree on a specific fix with an owner and due date, escalate what can't wait, and document it.
4. VerifyCheck the fix worked at the next contact. Trends tell you whether a site is getting better or needs escalation.

Demo data for illustration only. Sites, numbers, and scoring weights are fictional examples of a risk-based approach; a real study follows its own monitoring plan.
